Uned Cleifion sy’n Cyrraedd yw'r 'Uned Diwrnod y Llawfeddygaeth' sy'n paratoi cleifion ar gyfer eu llawfeddygaeth. Bydd y rhan fwyaf o gleifion yn achosion dydd, a byddent yn cael eu trosglwyddo o fan adfer i'r ward achosion dydd a elwir yn Ward Pasteur. 

Gall ward Pasteur fod yn brysur iawn, yma bydd cleifion yn cael eu rhyddhau yn ddiogel, a rhoddir cyngor a gwybodaeth gywir iddynt. Bydd cleifion sydd angen aros yn fwy nag un noson yn cael eu trosglwyddo i ward Llawfeddygol yn syth o Adferiad.

Gall yr Uned ddod yn brysur iawn, gan fod llawer o restrau theatr yn rhedeg ar yr un pryd, rhestrau bore a phrynhawn. Gall y Rhestrau Theatr amrywio o restrau Llawfeddygol, Orthopaedig, ENT, Colorectol, Gynae, Llawfeddygaeth y Fron, Fasgwlaidd a Thîm Poen.

Mae disgwyl i ddeilydd y swydd gyflawni gofal o bob math ac mae’n gyfrifol am asesu, datblygu, gweithredu a gwerthuso anghenion gofal cleifion ar ward Cleifion sy’n Cyrraedd/Pasteur.Bydd disgwyl i ddeilydd y swydd gysylltu â chydweithwyr nyrsio a meddygol ac aelodau eraill y timau Gofal Iechyd Amlddisgyblaethol i sicrhau bod gofal cleifion effeithiol yn cael ei ddarparu.

Bydd disgwyl i ddeilydd y swydd gymryd rheolaeth o'r Uned yn rheolaidd yn absenoldeb yr Uwch Brif Nyrs, a hefyd gweithio ochr yn ochr â'r Uwch Brif Nyrs. Bydd deilydd y swydd yn gyfrifol am baratoi rhestr staff a'i gweithredu, gan sicrhau bod digon o staff ar bob sifft, a sicrhau bod staff yn cael eu dosbarthu yn unol â'r galluoedd a llwyth gwaith er mwyn bodloni anghenion cleifion.

Arrivals is the ‘Day of Surgery Unit’ that prepares patients for their surgery. Most patients will be day cases and will be transferred from recovery to the day case ward called Pasteur Ward.

Pasteur ward can become very busy, here the patients will be discharged safely with the correct advice and support given.  Patients requiring to stay in for more than one night will be transferred to a Surgical ward directly from Recovery.   The Unit is  very busy as many theatre lists run at the same time, morning and afternoon lists.

The Theatre Lists can vary from Surgical, Orthopaedic, ENT, Colorectal, Gynae, Breast Surgery, Vascular and  Pain Team lists.

The post holder is required to carry out all relevant forms of care and is responsible for the assessment, development, implementation and evaluation of patients care needs on the Arrivals /Pasteur ward.  The post holder will be required to liaise with nursing and medical colleagues and other members of the Multi Disciplinary Health Care Teams to ensure the delivery of effective patient care.

The post holder will be expected to regularly take charge of the Unit in the absence of the Senior Sister and also work alongside the Senior Sister.  The post holder will be responsible for the preparation and implementation of the staff roster, ensuring that each shift is covered safely and ensuring that the allocation of staff is made according to the competencies and workload in order to meet patient’s needs.

The post holder will undertake staff supervision; attendance management and sickness reviews, individual performance reviews and personal development planning and ensuring issues of poor competence/ performance are addressed.  In the absence of the Senior Sister the post holder will be expected to take managerial responsibilities for the Unit and carry the bleep for the surgical wards.

The ability to speak Welsh is desirable for this post.
