Mae cyfle i unigolyn sydd ag angerdd am ddarparu gofal yn nes at gartref i ymuno â'n tîm fel Deietegydd Cymunedol Clinigol. Mae'n debygol y bydd y swydd yn cynnwys cymysgedd o glinigau cleifion allanol, ymweliadau cartrefi gofal ac ymweliadau cartref. Fodd bynnag, mae hon yn swydd gymunedol glinigol gyda gwahaniaeth. Fe gewch gyfle i ymuno â'r tîm therapi yng Ngharchar Berwyn un diwrnod yr wythnos. Byddwch yn rhedeg clinigau a chysylltu ag arlwyo ynglŷn â bwydlenni ar gyfer y dynion a welir mewn clinig, ond hefyd ar gyfer y carchar yn gyffredinol. Byddwch yn gweithio gyda'r tîm gofal iechyd i barhau â'r gwaith ardderchog a ddechreuodd deilydd y swydd flaenorol ar ddatblygu a chyfrannu at lwybrau cysylltiedig â maeth. Byddwch chi'n cael eich goruchwylio am y diwrnod hwn gan yr Arweinydd Clinigol ar gyfer Therapïau yng Ngharchar Berwyn, ond byddwch hefyd yn cael goruchwyliaeth a chymorth proffesiynol gan Arweinydd Tîm Cymunedol Clinigol a'r tîm Dieteteg yn Wrecsam a Sir y Fflint.

Mae’r gallu i siarad Cymraeg yn ddelfrydol ar gyfer y swydd hon; mae croeso i ymgeiswyr sy’n siarad Cymraeg a/neu Saesneg geisio am y swydd hon.

There is an opportunity for an individual with a passion for providing care closer to home to join our team as a Clinical Community Dietitian. The post is likely to include a mix of outpatient clinics, care home visits and domiciliary visits. However, this is a clinical community post with a difference. You will have the opportunity to join the therapy team at HMP Berwyn one day a week. You will run clinics and liaise with catering regarding menus for the men seen in clinic, but also for the prison as a whole. You will work with the healthcare team to continue the excellent work the previous post holder started on developing and contributing to nutrition related pathways. You will be supervised for this day by the Clinical Lead for Therapies at HMP Berwyn, but will also have professional supervision and support from the Clinical Community Team Lead and the Dietetics team in Wrexham and Flintshire.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
