This is an exciting opportunity for an experienced Registered Nurse who holds a DN qualification or is willing to work towards. Applicants without the qualification will be considered if they hold an MSc in teaching and develpment or Health & Social Care Management and Leadership.
The District Nurse Team Manager has continuous responsibility and accountability for the delivery and maintenance of high quality evidence based care provision;work autonomously in assessing, planning, implementing and evaluating care; provide specialist care advice to patients, colleagues and the wider multi-disciplinary team; carry out a variety of specialist nursing procedures; be responsible for funded nursing care assessments and joint assessments and care planning; To develop a culture of continuous improvement within the DN team ensuring safe and appropriate skill mix to meet patient care needs; lead and manage the community nursing team; delegate, support and supervise staff in the delivery of care.
