Permanent member of staff to join the CHC team in East, Flintshire/Wrexham. The post holder will manage on behalf of the East division the implementation, monitoring and reviewing of NHS Funded Registered Nursing Care (FNC) and Continuing NHS Health Care (CHC) in Care Homes (Nursing) and in the domiciliary setting to ensure patients receive care of a quality which meets best practice and national standards as prescribed and funded by the NHS.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
