An exciting opportunity to appoint a Mental Health Practice Development Nurse to support our East Continuing Health Care Team for older persons.  The successful candidate will play a crucial role in ensuring quality, safe, effective and well governed BCUHB funded care within these commissioned services.  A background in Dementia care so as to ensure we can meet the needs of our funded service user cohort within this area of speciality.  The role will require them to train as a Dementia Care Mapper to be able to deliver the quality assessments in line with the lived experiences of the service user and drive and improve quality from that perspective.
