An opportunity has arisen for a Band 5 Nurse based in Preswylfa, Mold. The aim of this post is to support improved service provision, to reduce unnecessary patient hospital admissions and support early discharge home from Hospital. The role of the Community Nurse in this setting is to work across both acute and community services, to support the early discharge and admission avoidance of patients who can be supported at home with a range of community services, focusing on reablement and patient stability. The hours of work are variable and will include weekend working. The geographical area of cover is across Wrexham and Flintshire area. Full induction into this post will be supported, with opportunities for further career development.
