This is an exciting opportunity for 2 experienced Registered Mental health nurse looking to join the community mental health team in Conwy on a fixed term contract or secondment. You will have excellent communication skills, be self motivated, proactive and committed to the promotion of innovation for service delivery.

The successful candidate will hold a complex caseload of individuals aged 18-65 with severe and enduring mental health issues and will work collaboratively to assess the needs and create care & treatment plans to meet the identified needs and risks identified. The post holder will be required to work independently and liaise with a variety of agencies and third sector organisations plus the individual’s carers and family members.

You will have exceptional communication skills and the ability to build and maintain effective working relationships, demonstrating a ‘can do’ attitude’.

The ability to speak Welsh for this position is desirable; both Welsh and English speakers are equally welcome to apply
