The Cardiac Rehabilitation team provides support and advice to patients, who have suffered a heart attack or who are undergoing investigations / treatments for heart disease, provide support to their families/significant others. We are based at BCUHB West Bodfan Ysbyty Eryri  we also Caernarfon and provide cover to Gwynedd and Anglesey visiting patients at their homes. We are looking to appoint an experienced, enthusiastic and well organised individual to a part time post initial with a view to increasing hours in the future, to help us maintain the high standard of care we provide in what can be a challenging environment. As a team member you will be responsible for your own case load, managing patients and acting as the patients advocate, liaising with other health care and allied professionals and pulling together patient information via PIMS and the clinical portal systems ensuring continuity of care. You will also be involved in the supervision of exercise assessment sessions and gym based exercise and educational classes

The individual we are looking for will have extensive experience in cardiology and would have ideally undertaken a cardiology course (e.g.ENB 124) and be ALS/ILS qualified. You must have or be willing to work towards the MSc Advanced Clinical Practice or the Non-Medical Prescribing course.

The team are part of National Audit of Cardiac Rehabilitation or (NACR)

The ability to speak Welsh is desirable but not essential for this post.
